A geriatric care manager is a professional, such as a social worker, counselor, nurse or gerontologist, who specializes in assisting elderly adults and their families attain the highest quality of life given their specific physical and/or mental circumstances.
Services Include:
Assess, arrange and monitor in-home help and other services
Review financial, legal or medical issues and other referrals to geriatric specialists with strategic mindfulness of possible future care needs and the preservation of financial resources
Provide crisis intervention
Assist with moving an elderly adult to/from a retirement complex, assisted living facility or skilled nursing facility
Offer compassionate counseling and support
Short-term or ongoing assistance for long distance caregivers or others requiring assistance
